A groundbreaking study conducted by the Centers for Disease Control and Prevention (CDC) has revealed a significant shift in drug consumption patterns, with smoking surpassing injecting as the most prevalent method of drug intake in overdose-related deaths in the United States. This comprehensive study, published on Thursday, stands as the largest investigation into the drug administration routes associated with fatal overdoses.

CDC Study Highlights Smoking Fentanyl Trend
The impetus for this study arose from concerning reports originating from California, suggesting a growing preference for smoking fentanyl over injecting it. The study's lead author, Lauren Tanz, a CDC scientist specializing in overdose research, emphasizes that while smoking fentanyl may pose a slightly reduced risk of fatality compared to injection, both methods carry a substantial overdose risk. The study underscores the need for further research to determine whether the shift towards smoking fentanyl translates into a reduction in overall overdose deaths in the United States.
Illicit Fentanyl: A Potent Driver of Overdose Epidemic
Illicit fentanyl, a highly potent synthetic opioid, has emerged as the primary culprit behind the escalating overdose crisis in the United States. This clandestinely manufactured substance, often mixed with heroin or other drugs, has contributed to a staggering number of overdose fatalities. Despite a slight increase in overdose deaths in 2022, following two significant surges during the pandemic, provisional data indicates a marginal rise in overdose deaths during the first nine months of 2023.
Smoking Fentanyl: Method, Advantages, and Risks
Traditionally, fentanyl was predominantly injected, but a growing trend has emerged among drug users to smoke it instead. Alex Karl, a researcher at RTI International who studies drug users in San Francisco, explains that individuals place fentanyl powder on tin foil or in a glass pipe, heating it from below to inhale the vapor. While smoked fentanyl is less concentrated than its injectable counterpart, some drug users perceive advantages to this method.
One notable benefit cited by users is the avoidance of pus-filled abscesses and the reduced risk of infections such as hepatitis. Karl recounts an instance where a drug user proudly displayed his arms, expressing relief at being able to wear T-shirts and secure employment without the stigma of visible track marks.
CDC Study Methodology and Findings
The CDC investigation utilized a comprehensive national database compiled from death certificates, toxicology reports, and records provided by coroners and medical examiners. Data from the District of Columbia and 27 states spanning the years 2020 to 2022 were deemed suitable for analysis, representing approximately 23% of the total overdose deaths in the United States during that period.
The study revealed a significant increase in the percentage of overdose deaths with evidence of smoking, rising by 74% between early 2020 and late 2022. Conversely, the percentage of deaths associated with injection declined by 29% during the same period. Although the number and percentage of deaths involving snorting also increased, the rise was less pronounced compared to smoking-related fatalities.
Challenges in Determining Exact Percentages
Experts acknowledge the complexity of precisely determining the percentages of deaths resulting from smoking, injecting, snorting, or swallowing drugs. In many cases, individuals may have consumed multiple drugs through different routes, while in others, no specific drug-taking method can be identified. Nonetheless, the study provides valuable insights into the evolving trends in drug administration methods.
Smoking as the Most Common Route in Certain Regions
The study's findings indicate that smoking emerged as the most prevalent route of drug intake in the Western and Midwestern regions of the United States. In the Northeastern and Southern regions, smoking was roughly on par with injecting. These regional variations highlight the diverse patterns of drug use across the country.
Study Limitations and Considerations
While the study offers valuable insights, it is not without limitations. As Kral points out, ascertaining the exact circumstances and reasons behind an overdose death can be challenging, especially in the absence of witnesses. Additionally, the detection of smoking-related deaths may be influenced by the presence of pipes or foil at the scene, which may not always be accurately documented.
Furthermore, Kral notes that many individuals who smoke fentanyl employ straws to inhale the vapors, which could potentially be mistaken for snorting by investigators. These factors underscore the complexities involved in accurately capturing the nuances of drug-taking behaviors and their associated risks.
The CDC study's findings underscore the evolving landscape of drug use in the United States, with smoking emerging as the primary method of drug intake in overdose deaths. While this shift may have implications for overdose risk and prevention strategies, further research is warranted to fully understand the consequences and implications of this changing trend.
