21 naval accidents in 3 years; corrective measures taken: Parrikar

Bengaluru, Feb 24: Defence Minister Manohar Parrikar gave a detailed report and the status of inquiries into various accidents involving Indian Navy's assents in the last three years.

According to Parrikar, there have been 21 accidents involving Indian Navy ships and submarines from January 2012 to January 2015.

Parrikar said that the lessons learnt from the reports of Board's of Inquiry (BoI) are implemented appropriately.

"Corrective steps have been taken by Naval Headquarters with extensive checks on weapon related safety systems and audit of Standard Operating Procedures on all operational naval units."

"Safety procedures and professional checks are re-emphasized. Incidents Study and Analysis Cells have been instituted at professional schools wherein lessons learnt are incorporated into professional training. Safety Audits of various units are being regularly conducted," Parrikar told in a written reply to H K Duain Rajya in Rajya Sabha.

He said the government has already approved a long-term submarine building plan. "Further, approval of necessity has been granted for life extension of six submarines," he added.

The status of inquiries into various accidents in the last three years are:

11.03.2012: Fire on board LCU L 38; 1 officer and 2 sailors were found culpable for incident and disciplinary action has been completed against them.

08.10.2012: Fire in engine room - INFAC 83; No individual or organization was held blameworthy for the incident.

19.12.2012: Fire in AFT battery pit onboard INS Shankush; No individual or organization was held responsible for the cause of fire and the incident was treated as an accident.

14.07.2013: Fire onboard INS Tarasa; 3 officers and 4 sailors were found culpable and disciplinary action against them initiated.

26.07.2013: Damage to Stbd Helo hangar and other fittings of INS Delhi by Yard 12701 (Kolkata); 2 officers were found culpable and disciplinary action against them has been completed.

14.08.2013: Explosion in INS Sindhurakshak; Board proceedings under examination at Naval Headquarters (NHQ).

22.09.2013: Fire onboard INS Viraat; 5 officers and 1 sailor were found culpable and disciplinary action against them has been completed.

04.12.2013: Fire onboard INS Konkan; 4 officers and 2 sailors were found culpable and disciplinary action against them initiated.

19.12.2013: Scraping of INS Tarkash shipside on SBW Knuckled; 2 officers were found culpable and disciplinary action against them completed.

23.12.2013: Collision of INS Talwar with unlit fishing boat: 4 officers and 2 sailors were found culpable.

Disciplinary action against 3 officers and 2 sailors completed. Summary trial of one officer is being undertaken at the Head Quarter Western Naval Command.

08.01.2014: Crack on sonar dome of INS Betwa; 3 officers were found culpable and disciplinary action against two of them completed and against one officer initiated.

17.01.2014: Suspended movement of INS Sindhughosh while securing to alongside berth; 1 officer was found culpable and disciplinary action initiated.

23.01.2014: Sea water ingress into tiller flat through a crack/hole in ship side of INS Vipul; 5 officers were found culpable and disciplinary action against them initiated.

30.01.2014: Damage to port propeller of INS Airavat whilst entering harbour; 2 officer was found culpable and disciplinary action initiated.

26.02.2014: Incident of fire in third compartment and death of two officers onboard INS Sindhuratna: 7 officers were found culpable and disciplinary action against them initiated.

07.03.2014: Accident onboard Yard 12701 (Kolkata) under construction at MDL while carrying out trials of engine room fire fighting system.

No individual or organization was held responsible for the cause of incident and the incident was treated as an accident.

06.04.2014: Incident of smouldering and thick smoke in armoury during Hot Work of INS Matanga; Board proceedings under examination at Naval Headquarters (NHQ).

28.06.2014: Damage to port propeller of INS Kuthar while coming alongside at Port Blair. Board proceedings under examination at Naval Headquarters (NHQ).

02.07.2014: Touching bottom of INS Cheetah while entering Kochi Harbour; Board proceedings under examination at Naval Headquarters (NHQ).

31.10.2014: Collision of INS Kora with MV Madeleine Rickmers at sea; Board proceedings under examination at Naval Headquarters (NHQ).

06.11.2014: Sinking of TRV-72; Board of Inquiry is in progress at Head Quarter Eastern Naval Command.

Another accident reported

In another accident involving a naval asset late on Thursday night (Feb 19), INS Sindhughosh suffered damages when a fishing boat hit its periscope during a special operation, off the Western coast.

The submarine was participating in Theatre Readiness Operational Level Exercise (TROPEX). A Navy spokesperson said that the exercise was conducted in pitch dark conditions as required by the exercise prerequisites.

"The submarine has returned to the naval dockyard at Mumbai and repairs would be undertaken soon," the spokesperson said.
