India Establishes First-Ever Guidelines for Seamless Organ Transport
The Union Health Ministry has introduced Standard Operating Procedures (SOPs) for the seamless transport of human organs across various travel modes, including air, road, railways, and waterways. This initiative aims to streamline the organ transport process, maximising the use of valuable organs and providing hope to patients awaiting transplants. These SOPs will guide organ retrieval and transplant institutions nationwide, ensuring adherence to best practices and quality standards.

Air Transport Guidelines
For air transport, airlines carrying human cadaveric organs can request priority take-off and landing from Air Traffic Control. They can also arrange front-row seats and priority reservations for medical personnel transporting organs. The source airport will communicate with the destination airport to facilitate the arrival process. The flight captain may announce that human organs are being transported during the flight.
To ensure smooth transport upon arrival, trolleys can be arranged to carry the organ box from the aircraft to the ambulance. If the ambulance is allowed onto the runway, airline staff can assist medical personnel in disembarking directly onto the waiting ambulance. Airport officials should establish a green corridor free from obstructions for transporting the organ box from the ambulance to the aircraft and vice versa.
Road Transport Protocols
The SOPs also outline protocols for transporting organs by road. A green corridor may be created upon request from specific authorities or agencies to facilitate organ transport. The One Trigger System allows a request from the organ allocation authority NOTTO/ROTTO/SOTTO to initiate this process. A nodal officer from the police department can be appointed in each state or city to handle issues related to creating green corridors, addressing jurisdiction, approvals, and security concerns.
Metro Transport Procedures
For metro transport, Metro Traffic Control should prioritise transit for metros carrying live human organs for transplant. Metro security staff should escort the clinical team carrying the organ box until they board at the metro station. A metro official can accompany the clinical team in the metro and cordon off necessary areas for the organ box. The Security Hold Area (SHA) should be informed about such transports to avoid delays in security checks.
Ensuring Safety During Transport
The SOPs emphasise that the organ box should be kept upright at a 90-degree angle during transport and labelled with "handle with care." It should also be secured with a seat belt for added safety. Similar guidelines have been issued for transporting organs by road, trains, and through ports via shipping.
Dr Anil Kumar, Director of National Organ and Tissue Transplant Organisation (NOTTO), highlighted that transporting a live solid organ is critical due to its limited shelf life and requires coordination between various agencies. He noted that organ transport has been identified as a challenge under the National Organ Transplant Program.
The SOPs were developed in consultation with NITI Aayog, concerned ministries, and transplant professionals. By implementing these guidelines, India aims to improve organ transport efficiency and save more lives through timely transplants.
