    3. Submit the completed application along with all the required documents to the office of the concerned District Agriculture Officer.
    4. Ensure that your application is submitted within 150 days from the date of the accident.

    • 'Baydhari Patrak' (Indemnity bond) in the prescribed format.

    • If the claimant is a legal heir (son/daughter) of a deceased registered farmer, proof of heirship like Pedhinamu or succession proof from the registered farmer (father or mother) is required.
