New Delhi, Nov 21: On Sunday, India witnessed the horrible Indore-Patna Express derailment near Kanpur in Uttar Pradesh.
The tragedy left 121 dead and more than 200 injured.
Every year, the country witnesses at least one major train accident that kills scores of people.
According to the Indian Railways, the main cause of train accidents is due to human failure.
In fact, human failure amounts to 87 per cent of accidents that occur on Indian Railways every year.

Out of the 87 per cent--44 per cent of the accidents are caused due to the failure of the railway staff and 43 per cent due to the failure of people other than members of the railway staff.
Moreover, two per cent accidents are due to failure of equipment, two per cent are caused due to sabotage, six per cent accidents are due to incidental factors, and the causes for three per cent accidents could not be established or the cases are still under investigation.
