The accident occurred on June 2, when three trains collided in Balasore district, killing more than 280 people and injuring over 1,000. The Coromandel Express entered the passing loop instead of the main line near Bahanaga Bazar railway station at full speed and collided with a stationary goods train.

The CRS is one of two agencies investigating the accident. The other agency is the Central Bureau of Investigation (CBI). Since the accident, the Railways has transferred several top officials of the South Eastern Railway under whose jurisdiction the tragedy occurred.

Initial investigations have suggested that a signalling interference, either by negligence or by intent, was the probable cause of the accident. The CRS report is expected to provide more details on the cause of the accident and the role of any individuals or organizations in the tragedy.
