Human Error Cited as Cause of Gen Bipin Rawat's Fatal Chopper Crash on December 8, 2021

The tragic helicopter crash on December 8, 2021, claimed the life of India's first Chief of Defence Staff, Gen Bipin Rawat. The accident involved a Mi-17 V5 helicopter and occurred near Coonoor in Tamil Nadu. Gen Rawat's wife, Madhulika Rawat, and several armed forces personnel also perished in the crash. A parliamentary panel report has attributed the cause to human error.

Gen Rawats Chopper Crash Caused by Human Error

The Standing Committee on Defence presented a report in Parliament detailing Indian Air Force aircraft crashes during the 13th Defence Plan period. The report recorded 34 crashes, including nine incidents in 2021-22 and 11 in 2018-19. The data included a column specifying the cause of each accident, listing human error as the reason for the Mi-17 crash on December 8, 2021.

Human Error Identified as Cause

The report highlighted that the Mi-17 crash was due to "HEA" or human error by the aircrew. This finding was part of a broader analysis of accidents involving IAF aircraft. The defence ministry informed the committee that they conducted 34 inquiries into these incidents during this period.

The ministry explained that recommendations from these inquiries are comprehensive. They cover process, procedure, training, equipment, culture, operations, maintenance, and administration to prevent future accidents. The Chief of the Air Staff's remarks on remedial measures are mandatory and have mostly been implemented.

Implementation of Safety Measures

While most recommendations have been actioned, some are still being implemented. The ministry assured that all necessary steps are being taken to enhance safety and prevent similar occurrences in the future.
