Human Error Behind CDS Bipin Rawat's Fatal Helicopter Crash: Official Report

Former Chief of Defence Staff (CDS) General Bipin Rawat, along with 12 others, tragically lost their lives in a Mi-17 V5 helicopter crash on December 8, 2021, near Coonoor in Tamil Nadu.

The incident, which also claimed the life of General Rawat's wife, Madhulika Rawat, has now been officially attributed to "Human Error (Aircrew)" by the Standing Committee on Defence.

Report Highlights

In a report tabled in the Lok Sabha, the committee clarified that the crash occurred due to aircrew errors during the flight. The report forms part of the findings from the Thirteenth Defence Period Plan (2017-2022), during which a total of 34 Indian Air Force (IAF) accidents were recorded, nine of which occurred in the financial year 2021-22.

The preliminary investigation revealed that the crash was triggered by the helicopter's unintended entry into clouds caused by sudden weather changes in the valley. This led to spatial disorientation of the pilot, resulting in a Controlled Flight Into Terrain (CFIT) scenario.

What Happened on December 8, 2021?

The Mi-17 V5 helicopter was en route from the Sulur Air Force Base in Tamil Nadu's Coimbatore to the Defence Services Staff College in Wellington. Tragically, the aircraft crashed into the hills just minutes before its scheduled landing.

The crash claimed the lives of all onboard, including:

  • General Bipin Rawat
  • His wife, Madhulika Rawat
  • 11 armed forces personnel

Group Captain Varun Singh, the lone survivor initially, succumbed to his injuries a week later at a military hospital in Bengaluru.

Key Findings of the Investigation

The investigation team, after analyzing the Flight Data Recorder (FDR) and Cockpit Voice Recorder (CVR), concluded the following:

  • The crash occurred due to an unexpected weather change that led the helicopter into clouds.
  • The pilot experienced spatial disorientation, resulting in CFIT.
  • Witness testimonies and additional evidence corroborated these findings.

Impact and Safety Measures

The report has raised concerns over pilot training and operational protocols. It underscores the critical need for enhanced safety measures, particularly in adverse weather conditions, to prevent future tragedies.
