The temple's legend is associated with the Hindu epic Mahabharata. Arjuna, the third Pandava, performed severe penance to propitiate Lord Shiva. Pleased with Arjuna's devotion, Shiva, accompanied by his consort Parvati, set out to meet Arjuna. Shiva disguised himself as a tribal hunter named Kiratha. A battle ensued between Arjuna and Kiratha, ultimately resulting in Kiratha's victory. Arjuna then realized that Kiratha was none other than Lord Shiva. Pleased with his devotion, Shiva granted the Pashupatha Arrow to Arjuna. Centuries later, a boy named Devala, cursed by a sage, worshipped the lingam made by Arjuna and was redeemed from his curse. The place was then named Rishnagakulam, and the temple was constructed by the public.

The presiding deity of the temple is Shiva in Gourisankara form, located in the main sanctum sanctorum, facing west towards the Arabian Sea. The Lingam in the main sanctum sanctorum is considered to be Swayambhu (divinely derived). On the northern side of the main sanctum sanctorum, there is a small shrine of Kirthamoorthy, believed to be the original Lingam worshipped by Arjuna. On the southern side, there is a small shrine for Shiva's son Ganesha. There is also a small area behind the main sanctum sanctorum, considered the shrine of Shiva's consort Parvati. Outside the inner temple circle, Ayyappa and Nagaraja are worshipped in shrines.
