To renew the passport you will need a copy of your old passport
You will need a self-attested photocopy of the first two and last two pages of ECR/Non ECR page, page of validity of extension if any
You will need a page of observation, if any made by the Passport Authority and No Objection Certificate or Prior Intimation letter.
For re-issue due to lost, stolen or damaged passports you will need proof od date of birth, proof of present address, police report in original
Your will also need an affidavit stating how the passport was lost, stolen or damaged and No Objection Certificate or Prior Intimation letter.
For damaged passport with passport number readable, name legible and photograph intact, you need proof or birth and affidavit stating how the passport was lost, stolen or damaged
To change name in passport you need fresh ID certificate in changed name and Gazette notification changing name in applicant’s department
For change in appearance in passport you need documents with Recent photograph showing latest appearance
For change in in address and date of birth in passport you need documents with proof of present address and proof of date of birth
